Requesting Former AA Medical Records

Employee medical records from former AA Medical can be requested by the employee through either a written or emailed request.

Requests should include the following information:

• Employee/former employee name
• Employee/former employee ID #
• Method by which record should be returned -- FedEx or email.
  • If choosing FedEx, must include an appropriate physical address (no PO boxes)
  • Ability to return using email may depend on the size of the file.  Note: emailed files will not be encrypted and there is some level of risk that the information in the email could be read by a third party.
